IMPORTANT INFORMATION REGARDING ACCESSING THE NEW WEIGHT LOSS JAB
Please be aware there is no need to contact your GP surgery regarding the weight loss jab. At Cook Lane Surgery we do only have a very small amount of patients that would be eligible and we will be in touch with those patients to offer this service. Questions and answers about tirzepatide (Mounjoro) :: West Yorkshire Health & Care Partnership
- Mounjaro is being introduced in a small number of GP practices as part of wider support for people living with obesity
- It is only available to a limited number of patients in the first year, based on strict NHS criteria
- Eligibility includes a BMI of 40 or more and at least four serious weight-related health conditions
- Patients must also take part in a structured support programme (wraparound care) including healthy eating and physical activity
- There is no need to contact your GP practice – eligible patients will be contacted directly by their practice or the NHS

From 26th March 2024, Cook Lane Surgery will have a new phone system. Patients will now be able to choose an option to direct them to the correct service. This will enable all patients contacting the surgery ease of access and a more efficient phone service.
Please remember to choose the correct option for your needs. The phone service will also allow patients to hold their place in our queueing system if there are more than 10 callers at any one time. We ask for your patience during the transition process. Thank you.
From November 1st 2022 all patients will have access to their own online medical records.
If you require online access please call the surgery on 01924 402073 or call in to the practice and ask our reception team about the process. It's quick and easy and allows you to view your medical records online.
You will need to be able to access the NHS app for this service.

Did you know you can attend your local pharmacy for minor ailments including:
- UTI's in females 16-64 years
- Impetigo adults and children over 1 year
- Acute Otitis ( ear ) children age 1-17 years
- Acute Sore Throats children and adults over 5 years
- Cold and Flu Symptoms
- Infected Insect bites adults and children over 1 year
- Hayfever
- Shingles adults over 18 years only
- Acute Sinusitis adults and children over 12 year
Please visit your local pharmacy for any of the above.
Please do not visit the surgery if you are experiencing Diarrhea and Vomiting symptoms. This is to reduce the risk of spreading infection to other vulnerable patients and our clinicians. Please call us and we may be able to help.

PREGNANT LADIES, 2-3 YEAR OLD, 16-18 YEAR OLD ( WITH UNDERLYING CONDITIONS) CAN BOOK INTO SEPTEMBER CLINICS.
EXTENDED ACCESS APPOINTMENTS
Please ask about our extended access appointments which can be on a weekend or on an evening. Call us on 01924 402073 to find out more details, or visit the appointments tab at the top of this page to find out more info.
We also now have access to extra appointments daily which are via telephone or Face to face. This service is run via Curo Health and enables us to offer more appointments on a daily basis. Please ask at reception about an appointment if you feel this is what you require. All face to face appointments are now available to pre book or book on the day from 8am.

The premises has a disabled access via the front automated doors which has double width to accommodate wheelchairs.

Between 6.00pm-6.30pm, this practice is covered by our own trained care navigators. They can provide advice and navigate you to the correct services. Please call our usual number 01924 402073 to use this service.
After 6.30pm when the practice is closed, 111 is the NHS non-emergency number. It’s fast, easy and free. Call 111 and speak a highly trained adviser, supported by healthcare professionals. They will ask you a series of questions to assess your symptoms and direct you to the best medical care for you, which may include booking you an appointment or arranging a visit from a GP outside our opening hours, although this will not be your usual GP. In a genuine emergency (threat to life) you should call 999. |
