Are you fit for travel?

Fitfortravel is a website provided by NHS Scotland.  It gives travel health information for people travelling abroad from the UK.  You can get vaccination, disease and malaria advice for your chosFit for Travelen destination.

 

Remember - always discuss your particular needs with the practice travel nurse at least 6 weeks before you travel.

Are you providing help and support to a relative, partner, friend or neighbour who could not manage without your help due to age, sickness, addiction or disability?Building Communities

You may not realise you are acting in a 'caring role’, let alone realise the benefits; whether emotional, physical or financial; available to help and support you in your role.
 
We have introduced a 'Registration of a Carer' form within the practice.  The form will allow us to note you are a carer on your records.  Our doctors will be aware of your role and are here to help. We can offer you an annual health check, annual flu immunization, arrange appointments to fit around your caring role, send repeat prescriptions direct to your pharmacy and provide information on access to practical and emotional support for you.  We hope you, our patient, will feel supported rather than isolated, in what can be a challenging, frustrating and stressful role. 

It is important to tell us and find out what help is available for you.  Please ask for a Carer Registration Form from Reception.

Advance/Anticipatory Care Planning

We want to be able to provide the best possible care for everyone.  To do this we need to talk about what is important to you and your family now, and in the future.
It is not always easy to talk about your views and how you would like to be cared for, if you have become sick or have a chronic illness.

What is Advance Care Planning?
Planning for the Future is usually called Advance Care Planning and it involves talking about what you want or don’t want to happen regarding the care you might need. Many people like to start thinking about issues to do with their care a long time in advance while they are feeling well. Other people like to wait until later.

What is an Anticipatory Care Plan?
Anticipatory care planning can help you to make positive choices and discuss how you can get support in the event of a flare up or deterioration in your health. It gives you the opportunity to think about, talk and write down your preferences and priorities for your health care. You do not need to do this if you do not want to.  A written plan is a summary of thinking ahead discussions between you and your carers ( family, friends and professionals involved). It is not a legal document but lets people involved in your care know about what is important to you.

Some aspects of Advance Care Planning:

  • Starting the discussions with your family or those involved in your care
  • Exploring options available to you and identifying your wishes and preferences
  • Refusing specific treatment if you wish to
  • Asking someone to speak for you
  • Appointing someone to make decisions for you using Power of Attorney
  • Talking about where you would want to be cared for in the future
  • Letting people know your wishes, hopes and fears

Connel Surgery

Surgery reception is open Monday to Friday between 08:00 - 18:00. Consultation times with doctors or nurses are as follows:

Surgery reception is open between 09:00 - 13:00 and 13:30 - 17:30 daily, 17.00 on a Friday, with the exception of Wednesday when the surgery is closed. Consultation times with doctors or nurses are as follows:

 Dalmally Surgery

Surgery reception is open between 09:00 - 13:00 and 14:00 - 17:00 on Monday and Friday.  Tuesday, Wednesday and Thursday reception hours are 09:00 - 13:00.

Please note there is no laboratory sample collection from Dalmally on a Thursday.

Patients may also find it useful to contact Dalmally Pharmacy for advice on minor ailments, medicines and other services.

Please note that although the times noted above are what we strive to provide, there maybe times when these change due to certain circumstances.
