National Day of Reflection - Tuesday 23rd March 2021  

Today marks the one year anniversary of our first lockdown. The National Day of Reflection is an opportunity for us to commemorate the tragic loss of life and to stand together with everyone who is grieving or has been impacted by COVID-19.  Over the last year the COVID-19 pandemic has brought so many changes to our lives.   The year has shown us what each one of us is capable of.   Amongst all the sadness there is also a lot that we should be grateful for.  As we look to the future, we will remember all those who are no longer with us, all the sacrifices made and all the challenges we have overcome.  Thank you for supporting the NHS.  This is our thank you to you.   #NHS #nationaldayofreflection #manorviewpractice #teammvp #weareallinthistogether

National Apprenticeship Week is a great opportunity to highlight the amazing opportunities that an apprenticeship brings to employers, individuals and our NHS.

Apprentice week is about highlighting the incredible work of our apprentices Matt and Lily who have contributed a great deal to the practice, patients and the community as a whole. During such unprecedented times they are an asset to our practice where they have worked with our multidisciplinary team and have been instrumental in the running of our community vaccination campaigns.

our practice now has a specialist mental health practitioner available to deal with your problems. please call after 11am in the morning for a appointment with Rachel Dean, the mental health practitioner. Rachel works alongside other members of the GP practice team.

She also has links and connections to the psychiatric department and councillors available for our patients. 

This is one of a new kind of service which we are offering to patients from July 2020.

It is with great sadness that we announce the retirement of Sheila who has dedicated 17 years of service at Askern Medical Practice which is at the heart of the community. Sheila will be very much missed by the staff, patients and the community. We thank you for your service and we wish you all the best for the next chapter of your life.

You said it, We did it!

just over a year ago we fully replaced our telephone system and further purchased software to monitor incoming calls. You had stated that service has improved since.

Moving forward, we have been using this software to assess the waiting times, phone call drops and time on the telephone. the receptionist telephone hub has been extended with staff providing phone service centrally branching out to all the sites and increased the number of staff answering from 4 members to 8 members of staff. All staff members have had training to provide the highest level of satisfaction. The further good news being the waiting times have dropped from 7 minutes to 4 minutes during peak times and between 1-2 minutes during off peak times. We have presented this at the patient participation group meeting held on the 4th of November which was widely attended on a virtual basis given we are in the second lockdown. 

Mexborough medical practice pilots 

the practice acts as a pilot for the following: 

- upto 60 additional appointments per week with Mrs Charlotte O'Reilly who is the muskuloskeletal practitioner at the practice. she has successfully completed her 6 month pilot and will continue to provide the service. 

- Mental health practitioner- we currently offer upto 45 additional appointments a week with Racheal dean who is an experienced psychiatric nurse. patients can be dealt with her for mental health issues. We have excellent feedback from patients who have used the above 2 services

they are the first of their kind in Doncaster GP practices and have been used as a template for recruitment in the primary care network practices from January 2021. We have shared the workings and the success story with 11 other practices in Doncaster and further afield. 

we are 100% compliant with '0 paper prescriptions'. All prescriptions have been done electronically to help with accuracy, speed and the green agenda and to reduce the infection spread by use of paper and handling.

Survey on use of technology in GP and nurse consulting in primary care- June 2020

given the nature of the high infection rate of COVID-19 and numerous deaths from it, the doctors have been advised by NHS England, RCGP and the British medical association on how services should be delivered. This meant a high proportion of services were being provided through the video and telephone consultations alongside some face to face consultations. 

there has been a big jump in these new forms of consultation. we have sent a text out to you on a survey linking to survey monkey and we thank you for all your responses. 

the results are as follows: 

60% of patients who responded were happy and felt that video and telephone consultations were adequate

10% of patients felt it made no difference

20% of patients did not like the new method of consultation, the reasons being cited were: difficulty in using technology, did not have a smartphone with camera, did not have adequate connection capabilities and some could not afford a mobile phone and had only access to a landline. 

this important group which constitutes 1/5 patients we felt were at a disadvantage and did not match up with stats nationally which quoted 3-5% of patients who didn't embrace technology. 

We believe we are the only practice in Doncaster who has done this survey and this has been passed onto the director of IT and director of primary care at Doncaster CCG to take note and create ways of helping these vulnerable patients.

World Patient Safety week- September 2020

The overall objectives of World Patient Safety week are to enhance global understanding of patient safety, increase public engagement in the safety of health care and promote global actions to enhance patient safety and reduce patient harm. The origin of the Day is firmly grounded in the fundamental principle of medicine – First, do no harm.

The COVID-19 pandemic is presently among the biggest challenges and threats to face the world and humanity, and health care is living its greatest crisis in patient safety ever! The pandemic has exerted unprecedented pressure on health systems worldwide. Health systems can only function with health workers, and a knowledgeable, skilled and motivated health workforce is critical for the provision of safe care to patients.

At the GP practice we have weekly meetings in addition to also having quarterly patient participation group meetings to constantly evaluate how we can improve patient safety and experience.

September 2020 is Childhood Cancer Awareness Month, and is symbolised by a gold ribbon, worn to commemorate the event. This is an annual international awareness month to raise support, funding and awareness of childhood cancers and the impact for sufferers and families of sufferers of childhood cancer. It was founded in 2010 by former US President Barack Obama and is mainly run in the form of awareness events by cancer charities.

Childhood cancer is devastating to growth and development in children, and devastating to their families and friends. At a time when young people should be focusing on school, play and socialising, if they have cancer, their focus can be on medication, operations, and what life they have left.

Parents of childhood cancer sufferers have the anguish of watching their children suffer, and the stress of decisions on medications and treatments as well as decisions on time of work for themselves and time off school for the child, and worst, trying to explain to a child what is happening to them and answering the resulting questions on life and faith, and worst of all is the loss of a child to cancer, a devastation that can never be put right.

Some forms of cancer are mainly or exclusively only seen in children, and that is something highlighted by the awareness month. Children can be more resilient to cancer and cancer treatments than adults, and there are many cases of triumph and complete recovery, where children make a complete recovery and grow up to normal life. But awareness, education and support are vital, which is why Childhood Cancer Awareness Month is also vital.

COVID-19 Business Continuity Plan

Introduction


COVID-19 has spread rapidly around the world in 2 months. The WHO have classified it as a public health emergency if international concern. The reproduction number is estimated at 2-3. The incubation period may be up to 24 days and it appears it may be possible for asymptomatic individuals to transmit the virus to others. It has high pandemic potential. GP practices have a duty  to protect their patients and a duty as employers to protect their staff. We all have a moral duty as brothers, sisters, patents, sons and daughters to protect our own families from our work related health risks. As independent contractors we need to be proactive in managing these risks for
ourselves rather than waiting for direction from others.


Plan
1. Notice on practice home page with brief information on our coronavirus plan and links to PHA website and 111 phone number

2. STOP notice on front door advising those with fever and acute respiratory symptoms not to enter but to return home and contact the practice by phone

3. Further Posters in the waiting room and notice on practice information screen re-iterating stop sign advice

4. No patients with acute viral symptoms (fever, cough, myalgia, diarrhoea) to be seen in the practice unless serious concern for their health (mainly age and co-morbidity, or if breathless).

a. As a large proportion of patients with URTI report SOB this should be     assessed on phone triage by ability to speak in full sentences and can be supplemented by questioning ability to climb stairs

b. As a major reason for seeing patients is to rule out bacterial infections/pneumonia and decline antibiotics, antimicrobial stewardship will be temporarily relaxed and antibiotics prescribed if felt appropriate by the GP and allows safe telephone only management of patient

5. Option to move to total triage by telephone due to concerns that there is now transmission in the community and possible transmission by asymptomatic cases

6. Need to explore remote working options for clinical and admin staff in case they need to self isolate for 14 days or longer – BSO remote access using own devices or buy cheap laptops; cheap contract or PAYG mobile phones for telephone contact or pay staff an allowance for use of personal mobiles for business use. Also useful if entering pandemic to protect staff
from exposure in the first place and keep them well and working


7. Suspend routine work like annual bloods and BP checks for monitoring repeat meds to create capacity and reduce footfall in the practice


8. Suspend QOF work and chronic disease management clinics (ideally with Dept Health agreeing QOF holiday for those that need it)

9. Choose a room in the practice to use as an isolation room

10. All scripts issued in 3 month supply to reduce demand and footfall

11. Appoint a COVID-19 business continuity lead and deputy

12. Ensure an up to date list of all staff contact details and NOK details

13. Plan in place to redirect phones and allow remote working of practice has to be closed for deep cleaning/decontamination

14. If remote working need system to get scripts printed and given to patients

In support of Carers and Alzheimer's week the practice are holding a carers morning at Denaby Medical practice on Thursday 6th June between 10am-2pm, and a Alzheimer's  morning on Monday 10th June between 10am-12noon at Conisbrough Medical practice - all patients and carers are invited to join the team.

Alzheimers week

This week is alzheimers week and the practice is working with Alzheimers society to support carers and patients with Alzheimers. We are having a drop in specialist Alzheimers support morning at the practice across all the sites, we will be writing to all our patients with dementia and their carers as well. 

Please use this opportunity to discuss and share any problems you have and advise us on how we could be a more dementia friendly practice.

carers morning: 

carers week is in mid June 2019 and we have been acclaimed for providing unfunded evening and Saturday appointments for carers since 2010. This has been innovative and considered as excellent in patient need responsiveness. we are holding a cafe and coffee morning during this week and we will be writing to carers asking them to attend. 

Patient participation group meeting- 

Our successfully established PPG meeting is in July 2019 and the next PPG speaker is a consultant ENT surgeon. we will also be discussing how we can help the gypsy and traveller population- Askern has one of the largest populations of this community in the UK. We will also be discussing patient surveys.

Mr Yousef (MPharm)

Mr Yousef has successfully completed 6 months at the practice dealing with complex medical queries and minor ailments. He has had very positive feedback from patients and is soon to be a dad!

PPG meeting - July 2018

Our second PPG meeting of the year has been very successful with patients from askern, mexborugh, conisborough and now joined denaby medical practice coming in for the meeting.

supporting local comprehensive school students-Matthew Clark- potential NHS Pharmacist of the future

the practice has a long standing policy of inviting students from local comprehensive schools who have the potential of being future NHS heros in various roles for example, paramedics, nurses, pharmacists, doctors and dentists. 

 

we believe if given the opportunity to train and prepare for interviews = they should be successful in entering these courses and we would hope they would come back to serve the local community. Matthew clark- 17 - son of a local police officer from the local comprehensive was given an opportunity to spend his work experience at the practice. this practice fully sponsored him in his training with the multi discaplinary team. matthew successfully completed his program and we wish him well in his application for university.

Practice has an ethos of supporting national charities working locally. Every year we have supported a charitable organisation- this year we had intended to do 2. Race for life ( as we have done over previous years)  in June 2018! And watch the space for another later in the year !!

Our practice’s clinical lead was invited to present to the rural university centre based in Lismore which
includes the University of Sydney, University of Newcross, University and Woologong. The speech was broadcast widely across to the outback to some of the Doctors working out there, talking about the various new models of care.


Visit from Kazakhstan

Visit from a leader of doctors in Kazakhstan. Damilya Negmonova attended the practice in June 2015 to see how the practice is run and learning from British care service to take back to Kazakhstan. Our staff had an excellent meeting with her and was delighted to exchange stories.

The Practice is approved by the University of Middlesex for training for the diploma in Minor Surgery and had the accolade of have various GPs coming to train under our clinical lead.

The practice has also trained Healthcare Assistants nationally and various lectures have been provided in Leicestershire and other areas for training.

The Practice has also done approved surgical training for level 1 and level 2 in a recent programme held in Manchester in early July 2015.

Vaccination Update- 08/06/21

AMP group practice will now be offering vaccinations in house at our Mexborough site for our patients. We are the first practice in Doncaster to gain this approval! Vaccinations will be for OVER 40s for their first dose OR anyone who has already had their first dose and is due for their 2nd dose (minimum 8 weeks from the first dose)

Latest Oxford AstraZeneca Vaccine Guidance 09/04/2021

  • Everyone who has had the AstraZeneca vaccine should still have a second dose of the AstraZeneca vaccine, irrespective of age, unless they have had a blood clot or have an existing risk of thrombosis (blood clotting)
  • People aged 30 and over or who have a health condition that puts them at higher risk of severe Covid-19 disease should still be offered the Oxford AstraZeneca vaccine. The benefits in protecting them against the serious consequences of COVID-19 outweigh any risk of this rare condition.
  • People aged 18-29 who do not have a health condition that puts them at higher risk of severe Covid-19 disease will be offered an alternative Covid-19 vaccine where available. (This has been recommended as a precaution as people under 30 are at less risk from Covid-19 and not because they are considered to be at particular risk of developing the rare blood clot.)
  • People under 30 can still choose to have the Oxford AstraZeneca vaccine if this will mean they can be protected more quickly and they have been made aware of the guidance.

NEW Vaccine Update 22/ 3/21

• National booking is now open for all aged 50 and over and teams are maximising  coverage of cohorts 1-9. Patients invited outside these cohorts is stated as only permissible in exceptional circumstances. YOU WILL BE CONTACTED BY THE SURGERY WHEN ITS YOUR TURN

Some patients have not been reached yet as some numbers are not obtainable as changed but the practice has not been informed and in other cases no response has been received.

If you belong to category between 1-9 (over 50's and 16+ WITH underlying health conditions) and haven't had your first dose please:

• A reduction in the supply of vaccine has been notified from 29th March 2021 for a period of 4 weeks a letter has been sent to vaccination sites about how to manage this situation (ie by closing unfilled slots in April for first doses)

• There is currently no contract in place to vaccinate below cohort 9, cohort 6 remains the current priority .

Further Government Guidance:

Starting the pausing of shielding arrangements from 1 April 2021.

• The home delivery service by community pharmacies and dispensing practices has been commissioned from 16th March to 30th June for anyone living in England notified by Test and Trace that they need to self isolate. They are only eligible for the service during the 10 day isolation period and first port of call should be family or friends.

1. residents in a care home for older adults and their carers
2. all those 80 years of age and over and frontline health and social care workers
3. all those 75 years of age and over
4. all those 70 years of age and over and clinically extremely vulnerable individuals

If you feel you are in one these groups, and have not had your 1st Vaccine, please telephone the practice after 2.30pm.

250% increase in staff answering the phone in the morning

Following your feedback in 2019 and 2020, we have now invested in new telephone systems and software and a huge number of care navigators at the practice. 

The software provides daily information to the telephone HUB leaders everyday. The average waiting times for December 2020 and the first week of January 2021 is between less than 30 seconds to 12 minutes. we are also happy with the feedback that you are providing with the ongoing independent patient survey showing that you are pleased when the telephone system states your number in the queue.

Community Surgery Update- Services resumed. 

We are glad to announce that community surgery and joint injections have now resumed. However we are operating on a limited service only due to government guidelines (which are continuously being updated) which we are adhering to. Services have been resumed adhering to strict national guidelines to ensure the safety of staff and patients. 

Anita Jackson Retirement- 34 Years in the NHS

It is with great sadness that we announce the retirement of Anita Jackson who has worked at Askern Medical Practice GP surgery for the past 34 years. During this time she has led the reception team and worked tirelessly to serve the community. During perhaps one of the most difficult periods for the GP practice and community in the early 2000's where there where no doctors, limited staff and no manager (the surgery was run by Doncaster PCT, 12 miles away) - she singlehandedly held the practice together. 

Her kind and joyful presence will be missed by not only the staff but also the patients who visit the surgery. We wish her all the best as she embarks on the next chapter in her life.

'I have worked within the NHS for 34 years, the past 17 years being with the Askern Medical Practice working alongside Professor Vijay Kumar  & Team from 2003 -  the practice formerly known as The Spa Surgery.

What a change I have encountered over those years!  From hand writing repeat prescriptions and running drop-in surgeries, sometimes not knowing how many patients would turn up to see a doctor, to learning a computer programme and printing prescriptions and booking appointments amongst other things.

Attending TARGET training sessions were always very interesting where we looked forward to meeting other practice teams along with some incredibly good training.

As Senior Receptionist I had lots of duties and responsibilities, too many to mention. The main ones being in charge of our team but most important of all to provide a friendly and efficient service to our patients.  I have also been a Fire Marshall and amongst other things most recently, was appointed Gypsy/traveller champion.

I have worked with some remarkable reception staff that I have mentored in their training.  I am lucky to have worked with them as they have remained my friends.

Of course there has been some stressful times during my time with AMP but the happy times outweigh them.  For instance our annual Christmas party where we got together with our branch surgeries and had such a good time, eating drinking and dancing. 

I have made several attempts to retire but always changed my mind knowing that I would miss my colleagues, doctors and our lovely patients, some of whom would come to me for advice or just a little chat.

Askern Medical Practice has been and always will be a major part of my life and as I embark on a new chapter, I am leaving with the fondest of memories.'

Community Surgery Update- Services to resume soon. 

We have received great demand for our community surgery and joint injections service which had to be put on hold due to COVID-19. We anticipate our services to resume soon, however we are waiting for further guidance. We shall keep you informed of updates. 

72nd Anniversary of the NHS 

This weekend, to mark the NHS’s 72nd anniversary, NHS organisations and staff will be taking the opportunity to say ‘thank you’ back to everyone who has supported them so far to respond to the Covid-19 pandemic.

On Sunday 5 July, the day the NHS was founded, the whole country is invited to come together at 5pm and applaud all those who have volunteered or helped keep the NHS and its staff going, and the commitment, courage and sacrifice shown by so many.

During these times of uncertainty across the UK, extraordinary people are making extraordinary things happen. Nationwide, nurses, doctors, carers, GPs and other healthcare and key workers are isolating from family and friends, re-training, re-deploying, volunteering and even coming out of retirement to join the effort to help tackle this pandemic – supported by members of the public who have been volunteering and donating food and other items.

The practice will resume some face to face appointments for blood tests and cytology. Please can all patients make sure they are wearing either a mask or a face covering for their face to face appointment as this is mandatory. 

Supporting Gypsy, Roma and traveller month

It’s Gypsy, Roma and Traveller History Month! Once again we celebrate the culture, history and heritage of these communities.   

During the pandemic we have been working hard to make sure that our traveller communities are supported through initiatives. 

- there have been texts sent to the community
- Vulnerable patients in that BAME community assessed and supported
- Social prescribing referrals done
- Regular & upscaled proactive care in place

Alert- possible cervical screening text messaging scam

We are aware of a possible scam. Some women are being contacted by text message, claiming to be from the call and recall service to advise they are overdue for screening. The message asks them to call a mobile number and provide personal details.
 These messages are NOT from the NHS Cervical Screening Programme.
 The Action Fraud Line has been contacted.
If you are unsure please call the practice to check if you are due for your cervical screening.

We held a fundraiser in aid of the MS society this was at our Mexborough site on Thursday 20th @ 9am - 1pm, this included a bake sale, guess the name of the teddy bear and a raffle to win a selection of Easter goodies all to raise donations for the charity.

Urgent Flash News
Due to severe floods in South Yorkshire, particularly around Rotherham, Mexborough, Denaby, and Conisborough areas, we are aware that many of our patients are unable to attend for their appointments today. 
Some of our staff who live in Sheffield and further afield have also been unable to enter the area in order to get to work.  However, the practice has put our  business continuity plan into place and we have a doctor on all sites dealing with emergencies.  We are fulfilling booked appointments and focusing on vulnerable patients.
A member of staff has been allocated to telephone some of our most vulnerable patients within the community, particularly those who are live close to the river, to ensure they are safe.
Another member of staff has contacted the local hospitals to make sure that elective appointments are being dealt with and to inform patients of any cancellations. 
We appreciate you bearing with us and we would like to thank all the staff who managed to come to work today despite the difficult circumstances, including one member of staff who visited three sites today to support members there.  We will ensure our staff get home safely this evening by providing transport where  necessary.

Reflection on week with AMP Healthcare

I was lucky enough to be able to spend a week with the staff across medical practices in Denaby, Mexborough and Askern. This was a great opportunity to improve my understanding of primary care provision, and see the potential for primary care to tackle the challenges currently facing health care providers. I was struck by the innovative procedures and practices provided across the practices. This included seeing minor surgeries, which included the removal of moles, skin tags, and providing therapeutic injections for a variety of conditions. Minor surgery in primary care is a cost-effective way of providing patient care, and was popular with patients, providing care close to home.

The friendliness and enthusiasm of all types and seniorities of staff was especially wonderful to see. Admin staff, practice managers, GPs, practice nurses, and healthcare assistants all shared a passion for providing high quality patient care. It was helpful to see the key role admin staff play in managing the patient’s journey, helping to book or cancel patients, keeping records up to date, and directing patients to the services most applicable to them.

Particular highlights of my week included the chance to see baby immunisations, where staff were comforting and supportive of the patient and parent; and seeing the provision of cervical smears, where clinician’s sensitivity and skill ensured patients felt supported and at ease. I saw first-hand the provision of patient choice, and the personalised relationships between the patient and clinician built over many years, enabling high-quality care.

Non-Medical support for the local community

Askern medical Practice - Doctors helping students from comprehensive schools to enter medical school

The practice have been forefront for many years in supporting students from perhaps disadvantaged backgrounds who are aspiring to study medicine. We altruistically offer time, effort and coaching for the students to help them obtain a place to study medicine. The practce has got a high success rate. The intention is to bring back Doctors to work in their own areas particularly this part of South Yorkshire which is very deprived and a hard to recruit area.

Michael Clarke Aspiring Pharmacist, Work placement

In November 2016, I was lucky enough to be invited to a work placement at all three of Professor Kumar’s Surgeries. During the period I was on placement I got to observe how a vast team worked together in a tight knit community and had the opportunity to spend time with every member of this essential team that provided a wonderful service to this local community of Askern Mexborough and Conisbrough.

My time at the surgery allowed me to observe how all of the healthcare professionals worked closely to the benefit of the community providing the best healthcare possible. I feel this was of great use to me as I am personally following a career path in to pharmacy role. I am currently in my last year of A-Levels, progressing in to university in the not too distant future. This was not only a educational placement but also offered me the opportunity to witness first-hand how best to deal with patient interaction, gain individual skills thus being something I can adapt and implement as I endeavour on my pharmacy career.

While providing excellent care to the community, Mr Kumar also relieves the NHS waiting lists by offering the facility to preform minor operations at some of his practices, another great experience I got to observe. My time throughout the placement was thoroughly eye opening and the knowledge gained invaluable from  the short period of time spent. It has also lead me to have a vast amount of respect for the work that goes into planning and providing the best care possible which is essential to the wider community.

I would also like to make a special mention to all the staff members, Such as the reception staff and the nurse’s that work tirelessly behind the scenes to allow for such a smooth operation to be carried out, as this would not be possible without their efforts, which also alongside the great work of professor Kumar allow for excellent care to be provided.

This work placement inspired me to follow a career path in to the medical route amongst the community. I hope that in the near future, as the pharmacy role is ever evolving, I am able to work side by side with professor Kumar again, Not only as a student but as a working healthcare professional, allowing for the continuation of his great work and dedication.

What joy it brings me to post about my two-week observership at the Mexborough and Askern Medical Practices, during my time in the UK! Having come from India, the opportunity to Deepthiobserve at the Medical Practices was a fantastic eye-opener to how Primary Care in the UK works.

I was thrilled to be able to sit in with the GPs and Nurse Practitioners during their consultations and get a glimpse of the types of cases that are dealt with in Primary care as well as the follow up care for the patients. I particularly enjoyed feeling comfortable enough to ask questions, both medically related and generic ones, to the medical personnel in between consultations.

The exciting hands on experience in the reception really helped me appreciate the administrative roles of staff; I got to assist with making QOF recalls, doing some coding and scanning and even helping to manage repeat prescriptions! Being present in a clinical meeting was another surreal experience that helped me understand how various issues were raised and dealt with by the team, both medical and administrative, at the practice. – The snacks provided were much enjoyed, too!

It was such a privilege to not only be observing here but also included as one of the amazing team, in the short time that I was there. I was delighted to talk about my experiences gained here, in my medical school interview and am very excited to be off to Liverpool to study medicine, in September! A huge and hearty thanks to all my lovely friends at Mexborough and Askern Medical Practices for helping me get one step closer to my dream!

    Tuesday 8.00 am  -  8.00 pm
  • Saturday mornings 09:00 – 11:30 (pre-bookable) on alternate weeks to our Mexborough Branch Surgery.

During Lunch between the hours of 13:00 – 14:00 please note the telephone is switched over to the emergency mobile telephone number which you can contact us on. However, the surgery is open, clinics are in operation and the reception is operational as normal.

There is emergency cover for medical emergencies only. To contact the surgery please ring 01302 700378 and you will be given a telephone number for medical emergency only.
