COVID19 Vaccines

The vaccine appointments are not dealt with by us, this service is carried out by a dedicated team. They will be telephoning patients and may also send an SMS text message containing a link in order to book your vaccine. You can also call 07517 556162.

Opting out of NHS Digital collecting your data (Type 1 Opt-out) – IMPORTANT PATIENT INFORMATION

The following link from NHS Digital explains why data is shared, it contains their transparency & privacy notices for doing so and also points to links where the patient can opt out if they want to: Opt out of sharing your health records - NHS (www.nhs.uk)

If you wish to register a Type 1 Opt-out with your GP practice before data sharing starts with NHS Digital, this should be done by returning the Type 1 Opt Out Form to your GP practice.

If you have previously registered a Type 1 Opt-out and you would like to withdraw this, you can also use the form to do this. You can send the form by post to The Surgery 1a Hawkesbury Road Canvey Island SS8 0EX or by email to cprccg.thesurgeryf81205@nhs.net

Once we receive your completed forms our reception staff will code the opt out accordingly.
