Breathe Easy Peer support Group Tower Hamlets

As Part of the drive to improve services for people with COPD and with the support of the British Lung Foundation we have started a New breathe Easy support

What are Breathe Easy groups?

  • A support group for local people affected by lung conditions including friends, family and carers.
  • Meet me once a month
  • Run by its members with support from the British Lung Foundation and myself
  • informal, friendly discussions in a supportive atmosphere
  • Regular talks from healthcare professionals and local services
  • refreshments, raffles social events and more.

Make new friends and learn more how to live well with a lung condition.

Prescriptions

We have suspended taking paper requests for prescriptions: therefor we have alternative methods for you to order your prescriptions as follows:

1) Sign up for online ordering at www.patientaccess.com or download the NHS app on your phone

2) Email the practice at thccg.limehouse@nhs.net - please provide your name, date of birth and required medication. Please DO NOT use this email for any other queries. Thank you. If you have been requested to send images by a clinician to this email address, please put the name and date of birth of the patient.

We have also suspended the collection of paper scripts from the practice. If you do not have a nominated pharmacy, please let us know what pharmacy you would like to collect your prescription from. If you do not nominate a pharmacy, we will use Nash Chemist on Commercial road.

Finally we request that you DO NOT ORDER your repeat medication if it is NOT due. If you do order medication and it is NOT due, it will not be issued. There are currently no medication supply issues.

We hope you understand the measures the practice has put in place to protect our patients and staff in this challenging time. We thank you for your co-operation.

 


EPS 

Have your prescription sent directly through to your pharmacy!

EPS enables your GP/Nurse to send prescriptions electronically to a pharmacy of your choice. If you would like more information on EPS please speak to reception.

In the future EPS will be the default option for prescribing and dispensing of prescriptions for primary care in England.

The practice would like to encourage more of our patients to take advantage of the benefits of EPSR2.

The NHS wants to give people better ways to see their personal health information online. We know that people want to be able to access their health records. It can help you see test results faster. It also lets you read and review notes from your appointments in your own time.

We're now letting you see all the information within your health record. If you are over 16 and have an online account, such as through the NHS AppNHS website, or other GP online services and apps, you will now automatically be able to see all future information entered on your medical record. Some people can already access this feature, this won't  change for you.

This means that you will be able to see records from your appointments, as well as test results and any letters that are saved on your records. This only applies to records from your doctor (GP), not from hospitals or other specialists, although you will be able to see any letters that hospitals or specialists send to your doctor (GP). You will only be able to see information from [insert date that your practice will make the change]. For most people, access will be automatic, and you won't need to do anything.

Your doctor (GP) may talk to you to discuss test results before you are able to see some of your information on the app. Your doctor (GP) may also talk to you before you are given full record access to make sure that having access is of benefit to you. If you are concerned that seeing your records is not right for you, you should speak to your practice.

These changes only apply to people with online accounts. If you do not want an online account, you can still access your GP health record by requesting this information via reception [or include online option]. The changes also only apply to personal information about you. If you are a carer and would like to see information about someone you care for, speak to reception staff.

The NHS App, website and other online services are all very secure.  You'll  need to make sure you protect your login details. Do not  share your password with anyone as they will then have access to your personal information.

If you do not want to see your health record, or if you would like more information about these changes, please speak to your GP or reception staff. You can also read more by going to www.nhs.uk/gp-record.

Please take note Saturday appointments are routine pre-booked appointments only, there are no urgent appointments available. If you require urgent medical advice please call the surgery and you will be transferred to the out of hours service. In a genuine emergency you should 999.Chest pains and or shortness of breath constitute an emergency.
