Community Pharmacy project- launch day 9th February 2022

We are participating in a new approach to improve access for patients to GP appointments. The aim is to direct patients to the most appropriate healthcare professional, which may be a GP or a pharmacist.

From 09/02/22 if your symptoms could be resolved by a booked consultation with the pharmacist instead of the GP, you will be given a same-day referral to a pharmacy of your choice.

We think this is a good thing. Once you see how great your local pharmacist is – they are highly trained and skilled clinicians experienced in treating minor illnesses – we don’t think you’ll look back.

This will also help us to free up GP appointments for people with more complex health needs and ensure that everyone gets treated at the right time, by the right healthcare professional.

APPOINTMENTS

You will be able to book a Doctors appointment one month in advance.

You can access allocated appointments via your online system, over the phone or at reception.

There will be a variety of 10 minute and 15 minute appointments to best suit your needs. 10 minute on the day appointments are handy for any on day problems that occur. This system will work alongside the old system whereby you can book triage, on the day, and Nurses appointments as per normal.

HERE Extended Hours Appointments

This is a pre –bookable NHS service you may have recently been offered an appointment at a Medical Practice to see a GP or other healthcare professional in the evenings or weekends.

Evening and weekend appointments are now available to book via. In order to utilise one of these appointments, you will be required to give consent for access to your notes to both read and add to them.

Appointment times will be between 18:30 - 20:30 Monday to Friday, Saturday 08:00 - 14:00 and 9:00 -13:00 Sunday’s.

Same day appointment will be available on weekends only Please call NHS 111 who may offer you an appointment at one of the Extended Hours hubs

Patient Confidentiality and Care Data

How information about you helps us to provide better care

Confidential information from your medical records can be used by the NHS to improve the services offered so we can provide the best possible care for everyone.

This information along with your postcode and NHS number but not your name, are sent to a secure system where it can be linked with other health information.

This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything.

If you wish to opt out you can go to www.nhs.uk/your-nhs-data-matters for more information and set a 'national data opt-out' or simply call 0300 3035678. Unfortunately this 'opt-ou' can no longer be set by a GP surgery.

GDPR

The General Data Protection Regulation (GDPR) is the new, Europe-wide law which comes into effect on 25th May 2018. It will be replacing the Data Protection Act 1998 in the UK. The GDPR is an opportunity for every organisation to give individuals (in our case patients) more control and rights over their personal data. The aim is to protect all EU citizens from privacy and data breaches in an increasingly data-driven world.

We recently had a CQC focused visit which resulted in Ardingly Court Surgery being given a 'good 'rating - CQC focused report

URINE SAMPLES

From now on we cannot accept urine samples unless requested by a health care professional. All patients must fill in a sample form (found at reception) or it will not be accepted. All samples must be brought to the surgery by 1 pm, we cannot process them after this time. Thank you.

*Team meeting/ opening times:

Every second Monday we have a team meeting between 8:30-8:45. This means that our doors open at 8:45 on these occasions (our phone lines are open as normal at 8:30).

Next meeting is 29/04/24 and then bi-weekly after this.

Telephone calls/ telephone service

We have installed a new telephone system to help us provide the best service possible. As part of these changes the majority of calls (in and out of the surgery are now recorded for training and monitoring purposes.

National capacity and appointment pressures:

Most GP surgeries up and down the country are currently reporting a very high level of demand, we are also finding this to be the case.  We are experiencing staff members being required to self-isolate and additional workforce pressure due increased appointment demand.  Our phone lines are also busier than usual.

We are doing everything we can to provide a safe and effective service to our patients.  We need to prioritise those most in need (such as young babies and extremely frail patients) and GPs are helping to make decisions regarding the priority of requests. You may need to wait longer than usual for an appointment or telephone call.

We ask that you are patient with us and work with reception staff so that we can find the right appointment and staff member to assist you or sign post you to the most appropriate support.

We have a zero-tolerance stance to abuse of any kind directed at any staff members (this includes verbal abuse, swearing, shouting or intimidation of any kind). You will be asked to register with another GP practice if you cannot abide to these terms.

Appointment system:

  • To mitigate risks and protect both patients and health workers, all online appointments are currently only available as telephone appointments. If deemed clinically necessary, you will be seen face to face after initial telephone triage has taken place*                                                                        

  • *Face to face appointments are, and have always been available at the surgery though out the pandemic. However, at present for some conditions and on day urgent appointments we will need to start with a telephone call. This is to minimalise foot fall to the surgery and the reduce the overall length of the appointment exposure time, protecting both patients and staff. This is in line with NHS Infection Control Guidance and ensures we are able to more readily support social distancing requirements within the building.

  • For pre bookable appointments (not urgent on day requests) you are able to choose face to face or telephone appointment for most conditions.  These currently cannot be booked online due to the complexities regarding different conditions/ reasons for requiring an appointment- you will need to; call, visit the surgery, or request a face-to-face appointment via our E-Consult banner at the top of the websites home page (this allows back information to be gathered for GP).  The reception team can advise you further regarding conditions that we can book face to face immediately (e.g. rash, breast exam etc) or advice regarding more complex requests.

  •  If you have any symptoms of flu (a new or persistent cough, fever or loss of taste or smell) we will arrange a telephone call for you. If you have hearing difficulties or are deaf, please let us know and will agree the best course of action.

In the first instance you must contact the service that delivered your vaccine. We do not have the computer system to make any changes to information held on NHS app.

Summary Care Record (SCR):

Access to SCR information means that care in other settings is safer, reducing the risk of prescribing errors. It also helps avoid delays to urgent care.

At a minimum, the SCR holds important information about;

  • current medication
  • allergies and details of any previous bad reactions to medicines
  • the name, address, date of birth and NHS number of the patient

Health partners 

Through our partnership with Brighton and Hove Federation, patients now have access to additional appointments from 18.30 to 20.00 weekdays and 09.00 to 17.00 Saturday and Sunday.  These include remote and face to face GP appointments as well as appointments with a nurse, pharmacist or health care assistant.  Specialist clinics are also being planned (e.g. menopause).  Please refer to the Federations website https://www.brightonandhovefed.co.uk/enhanced-access for updates.  Access to these clinics is by contacting the surgery in the normal way.   These appointments might be held at a different location to the surgery and are subject to availability.

Access for the Disabled

The surgery in Kemptown has no steps and is entirely on the ground floor. There is a toilet facility for the disabled. A hearing loop is available at reception and can be taken into your consultation too.
