Please be aware that sick note requests will no longer be available for request via our eConsult platform.
Patients that require a sick note are required to attend the surgery and complete a request providing inclusive to and from dates and your condition. You will also need to confirm your up to date contact number should a clinician need to contact you.
A GP may not always be the most appropriate person to see - eConsult helps us to know what you might need help for so that you can be directed to the right member of staff for the help you need first time around.
Should you have any queries please contact the surgery on 01495 244205
You can start your eConsult here eConsult
E-consult is an email option for contacting the surgery. We aim to respond by the end of the next working day. Please do not therefore use this method of contact for 'urgent' clinical requests. Please also note that this service is unavailable during weekends and bank holidays
