Care Navigation has been rolled out to GP Practices in the Derry Area

Our Multi-disciplinary Team is expanding in the practice and with this the role of Practice Administration staff is growing and changing. We now have a Pharmacist,Mental Health Worker and Social Worker on site to deal with your needs.Care Navigation will increasingly form part of the Practice receptionists’ role to ensure that all members of the GP Practice team are fully utilised and GP time is freed up for direct patient care. Care Navigation uses Signposting by our reception staff to help patients receive the right care from the right Professional at the right time.

Our next Bank Holiday Closure will be Mon 21st & Tues 22nd April 2025.

26/3/25 We are no longer able to accept Chemist Prescription requests on behalf of our patients from 1 May 2025. This is as per the Board Directive. Patients can contact the practice to get signed up for online ordering or they can leave their repeat order form in the surgery. There is a post box on the front wall for use during out of hours also.

PRACTICE MERGER

On 1/8/23 Bridge Street Family Practice patients transferred across to Foyleside Family Practice. We have now successfully merged and welcome our new patients.

CONSULTATIONS

We offer same day Face to Face appointments with a GPs , Advanced Nurse Practitioner  or a Mental Health Practitioner . You may also be offered an appointment with our Practice Nurse, Social Worker or a phone call from our Pharmacist. PLEASE ring as early as possible from 8.30am in the morning as once capacity is reached we will be dealing with urgent requests for the remainder of the day and you will be asked to ring back at another suitable time. There are some phone call slots for patients who decline a Face to Face appointment. We are constantly reviewing our appointment system to reflect demand.

Practice receptionists will take a brief history of the problem that you are calling about so that they can divert you to the most suitable member of the team to help you with that problem. All information is treated confidentially. Thank you for your cooperation.

Access

We have access to the surgery for patients in wheelchairs, and for patients with most other disabilities. Toilet facilities are provided for disabled patients.Most patient facilities are on the ground floor.

Opening Times

We are closed between 12.30-1.30 every day

We continue to take phone calls and deal with emergencies until 18:00. Access to a receptionist by telephone and face to face is available during these times.
